Required Forms

All Active Duty, Reserves, Disabled Veterans, Retirees, Dependents, and Medal of Honor Recipients are required to register their pet(s) at the Fort Liberty Vet Center. Registration is Monday-Wednesday, 9 am - 2 pm. Please allow 72 hours for complete registration paperwork. Registration Questionnaire (.pdf)
